Registration Process

1.Read the Dojo Etiquette and Uniform Requirements information below (review with your children if they are taking classes).

2. Fill out the form below and submit.

3. Submit payment to the dojo. Payments can be made in cash, check, VENMO (@mark-lake-9) or Zelle (904-314-0774).

4.After completing these steps, register your family or student with the Cuong Nhu Martial Arts Association. When you submit this form you will be directed to the Cuong Nhu “Join” page to follow the steps to register and pay online. This is an annual payment. If you have multiple members joining, you will have the option to add additional members to your profile once you complete the initial registration. Ask us at the dojo if you need any help with this. All students must have current dues paid with CNMAA to be eligible to test.
